대부분의 사람들은 통증이 생긴 뒤에야 치과를 찾게 됩니다.
하지만 통증이 느껴질 정도라면 이미 염증이 상당히 진행된 경우가 많습니다.

 

Most people visit the dentist only after they begin to feel pain.
However, when pain is already noticeable, inflammation is often significantly advanced.

 

초기 단계에서 정기검진을 받는다면 비교적 간단한 치료로 해결될 수 있지만,
치료 시기를 놓치면 주변 치아와 잇몸뼈까지 영향을 받게 됩니다.

 

If regular checkups are performed in the early stages, treatment can often remain relatively simple.
But when treatment is delayed, surrounding teeth and jawbone structures may also become affected.

 

오늘은 오랜 기간 어금니 문제를 방치하면서 다수 자연치를 상실하게 되었고,
임플란트와 보철 치료를 통해 기능을 회복한 증례를 소개해드리겠습니다.

 

Today, I would like to introduce a case in which long-term neglect of molar problems resulted in multiple tooth loss, later restored through implant and prosthetic treatment.

1. 구강 검진

1. Oral Examination

 

환자분은 어금니가 없어 식사가 어려운 상태였으며,
통증까지 동반되어 달서구 치과에 내원하셨습니다.

 

The patient had difficulty chewing due to missing molars and visited our clinic because of ongoing pain.

 

구강 내부를 확인해보니 이미 여러 어금니가 상실되어 있었고,
남아 있는 치아들 역시 심한 충치와 파절이 진행된 상태였습니다.

 

Upon examination, multiple molars were already missing, and the remaining teeth showed severe decay and fractures.

 

 

정면 사진에서는 치아와 잇몸 경계 부위인 치경부가 심하게 마모된 모습이 관찰되었습니다.

 

Frontal photographs also revealed severe cervical abrasion around the gumline area.

 

일부 치아는 치경부 마모가 매우 깊어 신경이 노출된 상태였습니다.

 

In some areas, the abrasion had progressed deeply enough to expose the tooth nerve.

 

정확한 진단을 위해 방사선 촬영을 진행하였습니다.

 

Radiographic imaging was performed for a more accurate diagnosis.

 

 

엑스레이 사진에서는 정상적인 저작 기능이 가능한 어금니가 거의 남아 있지 않았으며,
치조골 소실도 광범위하게 진행된 상태가 확인되었습니다.

 

The X-ray images showed that almost no functional molars remained, along with extensive alveolar bone loss.

2. 치료 계획

2. Treatment Planning

 

달서구 치과에서는 현재 상태를 충분히 설명드린 뒤,
환자분과 함께 단계적인 치료 계획을 수립하였습니다.

 

At our clinic, we carefully explained the condition to the patient and established a step-by-step treatment plan together.

 

 

보존이 어려운 치아는 발치를 진행하기로 하였고,
치경부 마모 부위는 레진 치료 및 신경치료 후 보철치료를 계획하였습니다.

 

Teeth deemed non-restorable were scheduled for extraction, while cervical abrasion areas were planned for resin restoration, root canal treatment, and prosthetic rehabilitation.

3. 진료 과정

3. Treatment Process

우선 통증을 유발하고 보존이 어려운 치아들을 발치하였습니다.

 

First, teeth causing pain and deemed non-restorable were extracted.

 

발치 후 즉시 임플란트를 식립하기보다는,
잇몸뼈가 자연스럽게 회복될 수 있도록 약 3개월의 회복 기간을 두었습니다.

 

Instead of immediate implant placement, we allowed approximately three months for natural bone healing.

 

이 기간 동안 남아 있는 치아들의 신경치료와 보존치료를 진행하였습니다.

 

During this healing period, root canal and restorative treatment were performed on the remaining teeth.

 

 

충분한 회복 기간을 거치면 염증이 안정되고,
치은과 치조골 상태 역시 보다 건강하게 회복될 수 있습니다.

 

With sufficient healing time, inflammation stabilizes and the gum and bone condition can recover more favorably.

 

비록 치료 기간은 조금 길어질 수 있지만,
그만큼 임플란트의 초기 고정력과 장기적인 안정성이 좋아질 가능성이 높습니다.

 

Although the treatment duration may become longer, implant stability and long-term prognosis often improve significantly.

 

 

염증과 골 소실이 심했던 부위는 골이식을 함께 진행하였습니다.

 

Areas with severe inflammation and bone loss required additional bone grafting procedures.

 

충분히 회복된 잇몸뼈 덕분에 임플란트 초기 고정력이 안정적으로 확보되었고,
1차와 2차 수술을 동시에 진행할 수 있었습니다.

 

Because the bone healed favorably, strong primary implant stability was achieved, allowing the first and second surgical stages to be performed simultaneously.

 

덕분에 추가적인 절개 수술 부담도 줄여드릴 수 있었습니다.

 

As a result, the patient could avoid an additional gum-opening procedure later on.

예상치 못한 변수

An Unexpected Challenge

 

치료 과정 중 신경치료를 진행했던 치아 하나가 파절되는 문제가 발생하였습니다.

 

During treatment, one of the previously root canal-treated teeth fractured unexpectedly.

 

해당 치아는 브릿지 계획에 포함되어 있었기에 환자분과 저 모두 고민이 많았습니다.

 

Since this tooth was originally planned as part of a bridge restoration, it became a significant concern for both the patient and the clinician.

 

 

환자분께서는 최대한 자연치를 살리고 싶어 하셨고,
저 역시 보존 가능성을 끝까지 고민해보기로 하였습니다.

 

The patient strongly wished to preserve the natural tooth, and we also decided to attempt every possible conservative option.

 

치아 내부에 포스트(Post)를 세워 치관 형태를 다시 회복해드렸습니다.

 

A post was placed inside the tooth to rebuild the crown structure.

이후 임플란트와 잇몸뼈의 안정적인 고정 상태를 확인한 뒤,
최종 보철 치료를 완성하였습니다.

 

After confirming stable osseointegration between the implants and jawbone, the final prosthetic restoration was completed.

Delayed Treatment Can Lead to More Complex Problems

 

이번 환자분은 다수 어금니 상실과 심한 치경부 마모 상태를 오랜 기간 방치하면서
치료 범위와 기간이 크게 늘어난 케이스였습니다.

 

This patient had delayed treatment for a long time despite multiple missing molars and severe cervical abrasion, resulting in a much more complex and lengthy treatment process.

 

 

하지만 단계적인 접근과 충분한 회복 기간을 통해
기능과 안정성을 회복할 수 있었습니다.

 

However, through a carefully staged approach and adequate healing time, stable function was successfully restored.

 

구강 질환은 미룰수록 치료 과정이 복잡해질 수 있습니다.
정기적인 검진과 조기 치료가 무엇보다 중요합니다.

 

Dental disease tends to become more complicated the longer treatment is postponed.
